You can go straight to 5.0.5... The only potential hiccup is that the list provided of updated files will be for 5.0.4 to 5.0.5 only...

This is when the examdiff or windiff utility comes in handy :)

Does doing an upgrade keep the customization we've done to the store? We've done a lot of hard coding on the store and we absolutely can't afford to lose the changes.

Not a chance...

You would need to diff() your files that were changed to ensure they were compatible and even then there's been some changes that completely deleted a file and moved the functions to another, etc... You are looking at a few hours if you want to upgrade a modified cart.
